Vietnam is located on the Indochina Peninsula in Southeast Asia. The country borders China to the north, Laos, and Cambodia to the west, the Gulf of Thailand to the south, and the Gulf of Tonkin and the Vietnam Eastern Sea to the east. Vietnam’s total length is 1,650 kilometers with a long coastline of 3,260 kilometers from Mong Cai in the north to Ha Tien in the south. The south of Vietnam is wider than the north.

Tay Nguyen (Central Highlands) is an exotic and off-the-beaten-track in Vietnam which is not well-known yet to foreigners. This region consists of five provinces: Kon Tum, Gia Lai, Dak Lak, Dak Nong, and Lam Dong. Endowed with the cool climate, you can escape from the heat of summers in Vietnam here. Southern Vietnam has warm weather all year-round with the average temperature of 26-28 degrees Celsius. This region has two seasons: rainy and dry and it will rain from May to the early November. Despite high temperature, the summer here is not as humid as it is in the North and there is no chilly winter in the south.

Travelling to Southern Vietnam , you can visit the metropolis Ho Chi Minh City with outrageous nightlife, beautiful Vung Tau and Phu Quoc’s beaches, and exciting Mekong Delta, one of two most arable areas in Vietnam besides the Red River Delta.

Southeastern Vietnam includes Ho Chi Minh City, Binh Duong, Binh Phuoc, Ba Ria – Vung Tau, Dong Nai, and Tay Ninh. Southwestern Vietnam, also known as the Mekong Delta region, includes 13 provinces: Can Tho, Ben Tre, An Giang, Hau Giang, Tien Giang, Ca Mau, Dong Thap, Long An, Vinh Long, Soc Trang, Tra Vinh, Bac Lieu, and Ca Mau. HISTORY Vietnam has history of more than 4 thousand years. Hanoi was found over 100 years ago while Sai Gon (Present-day Ho Chi Minh City) was established more than 300 years ago. When you travel from North to the South, you will travel along with the history of Vietnam.

CLIMATE Vietnam is a long country, the distance of Hanoi and Saigon is more than 2 thousand km. North of Vietnam has 4 typical seasons( Spring, Summer, Fall and Winter) . However, in the Central both extremes are amplified, with occasional snow in the winter and temperatures hitting 40°C (104°F) in the summer.

South Vietnam has 2 main seasons (Dry and Wet). Some traveler would like to start their Vietnam holiday in the North then end their trip in the South at some sunny beach resort before going home.

Located in the heart of Southeast Asia, Vietnam is a relatively small country, covering an area of 320,000 square kilometers. Its shape resembles that of an S or a dragon, exceptionally long and stretching nearly 2,000 kilometers from north to south. Its maximum width is only 600 kilometers in the northern part, while the minimum width narrows down to a mere 50 kilometers in the region of the Phong Nha National Park and underground caves in the central area. It shares its borders with China to the north, Laos to the west, Cambodia to the southwest, and is also bordered by the Pacific Ocean, specifically the East Sea, as well as the Gulf of Tonkin to the north and the Gulf of Thailand (or Siam) to the southwest.

Vietnam boasts about a dozen must-visit destinations throughout its expanse. The ancient millennium-old capital, Hanoi, the magnificent mountains, the famous Halong Bay, or its terrestrial counterpart in Ninh Binh are destinations not to be missed in the North (the upper part of the country). In the Central region, one should not overlook the former imperial capital of Hue and the old port town of Hoi An.

In the South, there are also essential stops for all itineraries, including the Mekong Delta. Especially the bustling metropolis of Ho Chi Minh City (Saigon) and the white sandy beaches of the islands of Phu Quoc or Con Dao. Since Vietnam shares its border with Laos to the west and Cambodia to the southwest, you'll easily have the opportunity to extend your journey to these two countries. From northern Vietnam , you can travel overland to Luang Prabang , the former capital of Laos. Additionally, from the southern Mekong Delta, you can journey upstream along the Mekong River to Phnom Penh , the Cambodian capital, and then continue your trip to Siem Reap, either by land or air.

With an estimated population of 98 million in 2021, Vietnam has lot of people. Its largest cities are Ho Chi Minh and Hanoi, respectively, each with nearly 9 million residents. Each offer a wide array of tourist attractions and local culture to appreciate.

  • Ho Chi Minh City – Formerly Saigon, this is Vietnam’s largest city, located in the south, with an estimated 8.993 million residents in 2019. Ho Chi Minh City is a fascinating city that never sleeps.
  • Hanoi – Vietnam’s capital city, located in the north has the second-highest number of people, at 8.054 million in 2019. Hanoi is full of historic tourist attractions and makes a great basecamp for exploring the north.
  • Hai Phong – It’s the third-largest city in terms of urban population located in the northern part of Vietnam. Hai Phong is known as “The City of Flame Flowers”.
  • Can Tho – The riverside city of Can Tho is famed for its floating markets and canals. It’s located a 4-hour drive southwest of Ho Chi Minh City. It’s the 4th largest city in VN.
  • Bien Hoa – The 5th largest city by population in Vietnam lies just 30 kilometers northeast of Ho Chi Minh City. Although overshadowed by HCMC, Bien Hoa has its fair share of tourist attractions.
  • Da Nang – The largest city in Central Vietnam at 1.231 million is a favorite of tourists and natives alike. Da Nang is the 6th biggest city by population, and close in proximity to Hoi An and Hue.
  • Buôn Ma Thuột – Located in Central Vietnam, Buon Ma Thuot is the country’s 8th largest city by population. It’s known as the “capital of coffee”. And is also the provincial capital of Dak Lak province.
  • Vinh – Nghe An’s largest city, Vinh sits on the coast in northern Vietnam. Thanks to the province’s natural beauty, tourism is growing steadily in town.

Vietnam’s Popular Beach Destinations

After spending a few days in the bustling streets of Hanoi or Ho Chi Minh City, tourists are probably eager to get away from the bustle. Head to the oceanside for some fresh air and beautiful sights. These costal cities are all popular tourists hotspots.

  • Da Nang – The fourth-largest city in Vietnam,  Da Nang is located in the central coast region. Hue lies north and Hoi An just south, combining to make a trifecta of travel.
  • Ha Long  – Two hours east of Hanoi (or four by bus) lies the mysterious Halong Bay (Ha Long Bay). Its unique limestone islands captivate visitors, as do its caves, beaches, and water sports. It’s best to spend at least one overnight on a boat cruise. Halong City offers a few five star hotels and various on-land amenities to go with the overnight river cruises.

  • Hoi An – Less than an hour south of Da Nang is one of Vietnam’s most sought after tourist destinations. It’s growing rapidly, as evidenced by the staggering number of luxury hotels and condominiums rising on the coast between Hoi An and Da Nang. Explore ancient ruins such as My Son Sanctuary.
  • Hue – The city of Hue itself doesn’t have any beaches, but it’s nearby some excellent resort beaches, which tend to be clean and quiet. Plan about 45 minutes from Hue to hit a good beach. The city itself is full of history, centered around the Imperial City.
  • La Gi – An off-the-radar coastal town in southern Vietnam, La Gi has a few good beaches on offer. South of it you can hit the famed Co Co Beach Resort and stay and play the night away.
  • Nha Trang – One of the top beach destinations in the country is Nha Trang. It boasts more than 535,000 residents, white sandy beaches, and a happening nightlife scene.
  • Phan Thiet – It’s dubbed as the “resort capital” of Vietnam because of Mui Ne beach . The city has several luxury hotels, resorts, and restaurants and has often been one of the top picks of foreign travelers.
  • Phu Quoc – Island off the coast of southern Vietnam, Phu Quoc is a paradise. Put this destination on your bucket list if you’d rather escape the busier beach spots Nha Trang or Hoi An.
  • Vung Tau – Probably the closest beach city to Saigon at around 3 hours, Vung Tau boasts a year-round population of over 527,000 residents. It has a few spots to enjoy the beaches and is an especially popular weekend spot from the biggest city in Vietnam.
  • Quy Nhon – Located in Binh Dinh Province, this coastal city in Central Vietnam has around 650,000 residents, with a lot of fishing and farming as its primary economy. Because of its lovely beaches, it’s shifting more towards tourism as well.
  • Rach Gia – With little more than 200,000 people, Rach Gia is a small rice processing town located on the southeast coast. While there aren’t notable beaches here, it’s used as a transfer point to nearby Phu Quoc Island and its beautiful beaches.

Vietnam’s Mountain Getaways

It’s not big cities and sandy beaches, Vietnam has its highlands too. Escape to mountain towns like Sapa and Da Lat to beat the heat of the lower lands. The northern portion of the country contains the highest mountains, which reaches 10,325.7 ft (3,147.3 m) in elevation.

  • Da Lat – More than 400,000 locals call the mountain town of Da Lat home. It’s located at 1500 meters (4,921 feet) with beautiful views, fresh air, and charming residents.
  • Sapa – Exactly like Da Lat, Sapa sits at 1,500 meters (4,921 feet). Except the views are even more mountainous as Sapa sits in the shadows of Fansipan. This is Vietnam’s largest peak at over 10,000 feet.

Vietnam’s Islands Retreats

Island life in Vietnam is possible. For those looking for a “busier” island experience, consider the clear coastal waters that surround Phu Quoc Island. This large paradise has a slew of accommodations, hotels and adventures to find. Small island getaways include Con Dao and the Cham Islands.

  • Cham Islands – Just off the coast from Hoi An is the popular tourist spot. Escape the crowds of the Old Town with a day trip or overnight stay on Cham Island.
  • Phu Quoc Island – Tucked away off the southwest coast of the country, Phu Quoc Island is surrounded by the Gulf of Thailand. It has some of the best beaches in Vietnam and some international visitors can even fly directly into Phu Quoc International Airport and not needing a visa like they would the mainland.
  • Con Dao Island – Before Côn Đảo became the sought-after destination, it used to be called “ Con Son “, a prison complex in the 1800s, created by French colonizers. The island was also involved in the Vietnam-American war in the 1960s. When you tour the region, you’ll see an intersection of old, colonial structures inspired by French and American architecture.

Since the Côn Đảo islands stand in the region of the South China Sea, it also became a key pier during those centuries. However, despite its not-so-pleasant past, the region has made its name as one of the most breathtaking gateways in Vietnam.

Located in the Indochinese Peninsula's eastern edge, Vietnam covers an area of around 331,211.6 sq. km. The coastline of mainland Vietnam is about 3,260 km long. The country also includes several islands. 

Vietnam is roughly S-shaped running for a distance of about 1,650 km in the north-to-south directions. It is about 50 km wide at its narrowest region. 

Vietnam is a country of tropical lowlands, rolling green hills, and densely forested mountains. As observed on the physical map above, low-level land covers about 20% of the country. 

The Red River delta is fronted by hills that rise gently into the high mountains of the northwest.

The Annam Highlands (marked on the map above) covers much of the central landscape, and in the southern areas, the coastal lowlands and Mekong River Delta merge. 

A fertile and narrow coastal lowland extends south from the Red River Delta to the Mekong Delta. The Mekong Delta is a low-level plain, one inundated by hundreds of small rivers and canals. Thick jungles and mangrove swamps cover the far-southern areas of land. 

The Red River (Song Hong), and the Mekong are the most significant rivers; both have numerous tributaries, and the latter is certainly among the great rivers of the world. 

As marked on the map above by an upright yellow triangle, Vietnam's highest point is Phan Xi Pang whose summit reaches 3,144 m; the lowest point is the South China Sea at 0m.

The provinces are subdivided into Provincial Cities, Towns, and Rural Districts (huyen). Hanoi, the capital and the second-most populous city of Vietnam is located in the northern region of the country.

The siren song of Vietnam has brought many travelers to its shores in search of incredible landscapes, pearl-white beaches , chaotic and vibrant cities, and one of the warmest welcomes in the world.

The tough decision for travelers isn't whether they should visit Vietnam, it's how they choose what to do with so many incredible options. Do you start in Hanoi or Ho Chi Minh City (HCMC)? Do you swap the big city experience for the beaches and seafood suppers of Phu Quoc island? Do you dive headlong into the history of a dozen rival empires, or embrace the dynamic, modern Vietnam that has emerged from the aftermath of conflict?

No matter what you want your vacation to look like, accept that you'll never cover everything in one trip and embrace it as a reason to return. Here are 15 incredible things to do that need to be on your Vietnam to-do list.

1. Embrace past and future Vietnam in historic Hanoi

The vibrant capital of the former North Vietnam – and today the capital of the nation – Hanoi is where old and new Vietnam come together. Traces of the imperial past float like ghosts in a city surging skywards – hawkers drift through the narrow lanes of the Old Quarter selling snacks from traditional yoke baskets, while the surrounding buildings are lit up with advertising displays and strip lights.

Exploring at street level is highly atmospheric and fantastic fun – mix up days visiting wartime and imperial relics with evenings feasting on some of Asia’s finest food and late nights dousing the sticky summer heat with glasses of bia hoi (local draught beer).

Planning tip: As they say, Hanoi rocks, and the best places to rock out like a local are lively live music venues such as the moody Binh Minh Jazz Club and keep-it-loud Hanoi Rock City .

2. Find your own island in the sun in Halong Bay and Bai Tu Long Bay

Okay, we concede that Halong Bay – the atmospheric sprawl of rocky coves and eroded karst islands to the east of Hanoi – is firmly discovered, but that doesn’t mean there isn’t more to discover away from the cruise ship crowds. If a tour on a diesel-powered luxury junk doesn’t appeal, consider hiring a kayak to potter around Lan Ha Bay near Cat Ba Island , where outcrops sculpted by wind and waves emerge from the water like the teeth of sea monsters.

Alternatively, leave the diesel fumes behind in calmer Bai Tu Long Bay to the north, where more jungle-capped islands shelter low-key resorts and languorous beaches.

3. Get your fill of imperial cuisine in handsome Hue

Vietnam’s Nguyen Dynasty held court in Hue until 1945, and the city’s damaged but still impressive historic relics feel somehow closer to imperial China than to the rest of Southeast Asia. However, for many travelers, it’s all about the food. More than half of Vietnam's estimated 3000 local dishes are believed to have originated in Hue, from the vermicelli-noodle-based bun bo Hue and crispy banh khoai pancakes topped with shrimp and pork to canapé-like, soft-and-crunchy banh ram it dumplings. The lavish imperial cuisine known as am thuc cung dinh – created by the emperors’ loyal chefs – can still be sampled in some of Hue's top restaurants, such as Thin Gia Vien and Hoang Phu .

Planning tip: Don’t restrict yourself to posh imperial banquets; for less than 100,000 dong you can gorge on tasty Hue staples at busy stalwart restaurants such as Madam Thu or at food stalls on the streets.

4. Meet Vietnam’s imperial leaders in the Hue tombs

Hue gets a second spot on the list thanks to the astounding imperial tombs, which preserve the mortal remains of a string of Nguyen emperors and empresses, from dynasty-founding Gia Long to Khai Dinh , the last Nguyen emperor to be buried on Vietnamese soil (his son, Bao Dai, the final emperor of Vietnam, was interred in the Cimetière de Passy in Paris ).

While crowds mob the Citadel and Imperial Enclosure, you can still find peaceful moments for contemplation by skipping the boat tours and renting a motorcycle to visit the extravagant mausoleums along the Perfume River. Don’t miss the towering  Thien Mu pagoda , looming over the north bank on the city fringes.

Detour: Equipped with two wheels, you can also buzz north from Hue to enjoy unspoiled dune beaches on the sandy barrier island stretching north and south from Thuan An.

5. Learn to make soups, stir-fries and salads on a cooking course

There’s no better souvenir to bring home from Vietnam than being able to prepare your own Vietnamese feast. Indeed, Vietnamese food arguably has a bigger dinner-party cachet than more familiar Thai cuisine.

Up and down the country, you’ll find cooking courses that start with a fragrant market trip to buy fresh ingredients and local herbs and end with a banquet of the dishes you’ve prepared, covering everything from summer rolls to pho noodle soup.

Hoi An is the most popular destination for aspiring chefs – small and personal Green Bamboo Cooking School is highly recommended. There are also good cooking schools in Hue, Hanoi and HCMC.

6. Unravel Vietnam’s complex religious history in My Son

While travelers mob the regal assembly halls, pagodas and historic homes of Hoi An, nearby My Son offers a calmer vision of Vietnam’s rich and layered past – particularly if you come in the afternoon. The big lure here is the atmospheric collection of UNESCO-listed Hindu temple ruins surrounded by jungle in a loop of the sacred Thu Bon river.

Sharing many architectural features with the Khmer temples of Cambodia and Thailand , these crumbling, red-brick shrines were built between the 4th and 14th centuries by the Hindu kingdom of Champa, whose descendants – the Cham people – can still be found in pockets in central Vietnam. 7. Visit the Cao Dai Holy See in Tay Ninh

One of the world’s youngest faiths, the Cao Dai religion was founded in the Mekong Delta town of Tay Ninh in 1926, fusing elements of ancestor worship, folk religions, Confucianism, Taoism, Buddhism and Roman Catholicism, with the ultimate goal of freeing the soul from the endless cycle of reincarnation.

To explore its complex customs, head to the cathedral-like Cao Dai Holy See in Tay Ninh. Inside, you’ll see weapon-toting statues, an all-seeing eye, and even a Communist-looking red star – the sect reveres, amongst other figures, Jesus Christ, Buddha, Mohammed and even French novelist Victor Hugo.

Planning tip: Visitors can enter the shrine to witness prayers four times a day; if you have time to kill before or after a visit, detour 15km (9.3 miles) northeast to Nui Ba Den mountain. You can take a gondola then hike to reach its summit cave temples, then whoosh back downhill on a luge-style slide.

8. Rumble through the highlands by motorcycle

Vietnam largely moves on two wheels, and a motorcycle is by far the most enjoyable way to explore the hill country of the northwest , even if it takes some courage to adjust to the driving conditions, the errant livestock, and the steep, winding roads. Rent a quality bike in Hanoi and pop your wheels on the train to Lao Cai, to avoid the nerve-jangling ride out of the capital.

Once you reach the heart of the hill country, you can rumble out to outlying villages far from the tourist crowds for epic views and nights in charming homestays run by members of Vietnam’s tribal minorities.

Planning tip: You’ll need an international driving permit to ride legally in Vietnam, and this is only available for some nationalities. Many travelers manage to rent a motorcycle without a permit, but if things go wrong, you won’t be covered by your travel insurance.

9. See the American War through Vietnamese eyes

Modern-day Vietnam is moving on from conflict, but the battlefields from the American War still linger as a sobering reminder of what people had to go through to get to this point. Sites of American losses such as Hamburger Hill in the Demilitarized Zone (DMZ) are etched into the popular consciousness, but the wartime tunnels at Cu Chi near HCMC and Vinh Moc near Hue offer a glimpse of the Vietnamese experience, as ordinary people went to extraordinary lengths to resist the world’s greatest superpower.

10. Visit villages in the "Asian Alps"

The former French Hill station of Sapa is surrounded by mountains so impressive that French colonizers called them the “Tonkinese Alps,” and the surrounding villages of the H'Mong and Dao ethnic minorities have become popular destinations for hikers – and somewhat commercialized in the process.

For a taste of the scenic serenity that first drew travelers to these lush green hills, head instead to drier, calmer Bac Ha , or the trails and mountain roads around Ha Giang province, tucked against the border with China and seemingly sculpted by the hands of the gods. Stay in hospitable homestays and explore markets, peaceful stilt-house villages, French-era relics and soaring limestone pinnacles, away from the maddening crowds.

Planning tip: There's a lot of competition for the title of best vista in Vietnam but the lookouts gazing over the Dong Van Karst Plateau are strong contenders. The area is designated as a UNESCO Geopark, and the trip from Yen Minh to Dong Van and over the Mai Pi Leng Pass to Meo Vac is particularly spectacular.

11. Find a perfect cup of coffee in the Central Highlands

It was the French colonizers who brought coffee from the Arabian peninsula to Vietnam, but it was the farmers of the Southwest Highlands who mastered the art of coaxing quality beans from these undulating hills. Rising to 1600m (5250ft), Dak Lak Province provides the perfect terroir for growing robusta beans, and the regional capital of Buon Ma Thuot is a great place to tour plantations and track down a quality cup of ca phe , particularly during the annual coffee festival in March. If you insist on arabica beans, head to Dalat and visit the community K'Ho Coffee cooperative, supporting local coffee growers from the K’Ho minority.

Planning tip: The best time to visit coffee country is from September to the end of December when the harvest season is in full swing and plantations are filled with baskets of red beans.

12. Enter the world’s largest cave in Phong Nha

Phong Nha-Ke Bang National Park in north central Vietnam is a lost world of jungles and caverns, including the world’s largest, Hang Son Doong. The scale of this wonder of nature is simply mind-blowing – a 747 airplane could fly through the cave’s main tunnel.

Parts of the limestone ceiling have collapsed, bringing in natural sunlight, so Hang Son Doong contains an entire rainforest ecosystem with flying foxes, rare langurs, and even a small population of tigers. Since 2012, one tour company – Oxalis Adventure – has been allowed to take a strictly limited number of visitors into Hang Son Doong on challenging four-day treks; the price is stratospheric but so is the scenery.

13. Walk with the ghosts of French Indochina in Ho Chi Minh City

The former capital of South Vietnam may have changed its name from Saigon to Ho Chi Minh City (HCMC) , but the past remains close to the surface, from the American War relics in the HCMC Museum and War Remnants Museum to the city’s elegant French-built mansions and civic buildings – many now housing museums, restaurants and boutique hotels.

To get a feel for vintage Saigon, stop for a cup of ca phe sua (milk coffee) at the elegant Hotel Continental Saigon , sample cutting-edge French cuisine at La Villa , and take a stroll past the Central Post Office, Notre Dame Cathedral and the People’s Committee Building – built as HCMC’s Hôtel de Ville but rebranded as the Vietnamese sent the colonizers packing.

14. Bask on beautiful beaches

It was the beaches of Phu Quoc Island and Danang – developed as a playground for GIs during the American War – that put Vietnam on the map for seaside vacations, but both have developed into lively resorts. To find unspoiled stretches of sand, modern-day castaways aim their sights at the kitesurfing beaches around Mui Ne , the island sands of the Con Dao archipelago and long, languorous Hong Van Beach on Co To Island in Bai Tu Long Bay .

Planning tip: The best time for a beach trip varies as you move around the country. In central Vietnam, skies are brightest from January to August, while December to April is the beach window on the south coast, and northern Vietnam sees plenty of dry days from October to April.

15. Float on the Mekong Delta

Reaching out into the East Sea like an enormous hand, the mighty Mekong Delta marks the end point of Southeast Asia’s longest river – a 4350km (2700 mile) monster, rising on the Tibetan plateau and emptying to the south of Ho Chi Minh City. This waterlogged wonderland is Vietnam’s rice bowl, nurturing a network of sleepy towns and stilt villages whose residents use the river as their primary artery for life and trade.

For comfortable exploring, book an overnight cruise along the main channel near Can Tho or a cross-border trip to Siem Reap in Cambodia. For a less commercial experience, take a day trip to the backwaters near Ben Tre with Mango Cruises or make your own arrangements with boat owners in Delta villages.

More than a lake, a capsule of tranquility before the noise of the city. This lake full of legends has a beautiful temple that you can visit, as well as proximity to the historic center of the city. You can picnic, exercise or interact with the inhabitants, watch them pass by, or try to beat them in a game of chess. The greenery of the trees adds simplicity and naturalness to an environment that struggles to survive despite environmental pollution. Some soft-shelled turtles can still be seen in the lake. Let some locals soak you in the myths and history behind this wonderful lake. Even on the shores of the lake there are several places to have lunch, and have a delicious coffee. As usual in communist regimes the practice of embalming has become almost obligatory by the time their top leaders move to another plane. Such is the case with Ho Chi Minh, the Vietnamese leader, whose body has been lying in this mausoleum since the year of his death in 1969. Located in the square of the famous independence meeting presided over by the leader, you will see that a large audience lines up to take a look at the embalmed body. All aware of the security measures that must be complied with in order to give a visit under a climate of respect and moderation. This temple of great importance to Vietnamese culture is a tribute to Confucius. Its antiquity adds an incalculable value, having as origin the year 1.070. Imperial City of Thang Long

Hanoi is an ancient city. Throughout its existence was residence and political seat for the various dynasties that reigned in the past. For this purpose, the Imperial City of Thang Long was made of palaces and green areas. It is believed that it dates from the year 1.010, as some clues found affirm. Yellow Fever Vaccine & Malaria Prevention Information, by Country

The following pages present country-specific information on yellow fever (YF) vaccine requirements and recommendations, and malaria transmission information and prevention recommendations. Country-specific maps are included to aid in interpreting the information. The information in this chapter was accurate at the time of publication; however, it is subject to change at any time due to changes in disease transmission or, in the case of YF, changing entry requirements for travelers. Updated information reflecting changes since publication can be found in the online version of this book and on the Centers for Disease Control and Prevention (CDC) Travelers’ Health website. Recommendations for prevention of other travel-associated illnesses can also be found on the CDC Travelers’ Health website .

Yellow Fever Vaccine

Entry requirements.

Entry requirements for proof of YF vaccination under the International Health Regulations (IHR) differ from CDC’s YF vaccination recommendations. Under the IHR, countries are permitted to establish YF vaccine entry requirements to prevent the importation and transmission of YF virus within their boundaries. Certain countries require proof of vaccination from travelers arriving from all countries ( Table 5-25 ); some countries require proof of vaccination only for travelers above a certain age coming from countries with risk for YF virus transmission. The World Health Organization (WHO) defines areas with risk for YF virus transmission as countries or areas where YF virus activity has been reported currently or in the past, and where vectors and animal reservoirs exist.

Unless issued a medical waiver by a yellow fever vaccine provider, travelers must comply with entry requirements for proof of vaccination against YF.

WHO publishes a list of YF vaccine country entry requirements and recommendations for international travelers approximately annually. But because entry requirements are subject to change at any time, health care professionals and travelers should refer to the online version of this book and the CDC Travelers’ Health website for any updates before departure.

CDC Recommendations

CDC’s YF vaccine recommendations are guidance intended to protect travelers from acquiring YF virus infections during international travel. These recommendations are based on a classification system for destination-specific risk for YF virus transmission: endemic, transitional, low potential for exposure, and no risk ( Table 2-08 ). CDC recommends YF vaccination for travel to areas classified as having endemic or transitional risk (Maps 5-10 and 5-11 ). Because of changes in YF virus circulation, however, recommendations can change; therefore, before departure, travelers and clinicians should check CDC’s destination pages for up-to-date YF vaccine information.

Duration of Protection

In 2015, the US Advisory Committee on Immunization Practices published a recommendation that 1 dose of YF vaccine provides long-lasting protection and is adequate for most travelers. The recommendation also identifies specific groups of travelers who should receive additional doses, and others for whom additional doses should be considered (see Sec. 5, Part 2, Ch. 26, Yellow Fever ). In July 2016, WHO officially amended the IHR to stipulate that a completed International Certificate of Vaccination or Prophylaxis is valid for the lifetime of the vaccinee, and YF vaccine booster doses are not necessary. Moreover, countries cannot require proof of revaccination (booster) against YF as a condition of entry, even if the traveler’s last vaccination was >10 years ago.

Ultimately, when deciding whether to vaccinate travelers, clinicians should take into account destination-specific risks for YF virus infection, and individual risk factors (e.g., age, immune status) for serious YF vaccine–associated adverse events, in the context of the entry requirements. See Sec. 5, Part 2, Ch. 26, Yellow Fever , for a full discussion of YF disease and vaccination guidance.

Malaria prevention.

The following recommendations to protect travelers from malaria were developed using the best available data from multiple sources. Countries are not required to submit malaria surveillance data to CDC. On an ongoing basis, CDC actively solicits data from multiple sources, including WHO (main and regional offices); national malaria control programs; international organizations; CDC overseas offices; US military; academic, research, and aid organizations; and the published scientific literature. The reliability and accuracy of those data are also assessed.

If the information is available, trends in malaria incidence and other data are considered in the context of malaria control activities within a given country or other mitigating factors (e.g., natural disasters, wars, the coronavirus disease 2019 pandemic) that can affect the ability to control malaria or accurately count and report it. Factors such as the volume of travel to that country and the number of acquired cases reported in the US surveillance system are also examined. In developing its recommendations, CDC considers areas within countries where malaria transmission occurs, substantial occurrences of antimalarial drug resistance, the proportions of species present, and the available malaria prophylaxis options.

Clinicians should use these recommendations in conjunction with an individual risk assessment and consider not only the destination but also the detailed itinerary, including specific cities, types of accommodations, season, and style of travel, as well as special health conditions (e.g., pregnancy). Several medications are available for malaria prophylaxis. When deciding which drug to use, consider the itinerary and length of trip, travelers’ previous adverse reactions to antimalarials, drug allergies, medical history, and drug costs. Yellow Fever Maps

2 In 2017, the Centers for Disease Control and Prevention (CDC) expanded its YF vaccination recommendations for travelers going to Brazil because of a large YF outbreak in multiple states in that country. Please refer to the CDC  Travelers’ Health website for more information and updated recommendations.

3 YF vaccination is generally not recommended for travel to areas where the potential for YF virus exposure is low. Vaccination might be considered, however, for a small subset of travelers going to these areas who are at increased risk for exposure to YF virus due to prolonged travel, heavy exposure to mosquitoes, or inability to avoid mosquito bites. Factors to consider when deciding whether to vaccinate a traveler include destination-specific and travel-associated risks for YF virus infection; individual, underlying risk factors for having a serious YF vaccine–associated adverse event; and destination entry requirements.
