New public guidance for testing in the NWT

Yellowknife — December 31, 2021

The NWT Chief Public Health Officer (CPHO) is immediately issuing the following testing guidelines for anyone who receives a POSITIVE rapid antigen test. These are the take-home tests that are provided at airports, through NWT schools, and through the DetectNWT program. Rapid antigen tests will also become available through health centres as soon as possible. They are currently available at the COVID-19 testing clinic in Yellowknife for those who meet eligibility.

The NWT is preparing for an influx of returning travellers in the coming weeks and therefore it is expected that more COVID-19 positive cases will result due to importation. Testing centres must prioritize laboratory resources to concentrate on highest-risk individuals and will no longer confirm most at-home positive tests.

Fully vaccinated individuals who receive a positive result from an at-home test will not need to repeat testing at a testing centre if they are at low risk for severe outcomes and have mild, non-serious symptoms. These individuals will be required to isolate for 10 days along with their household members and other close contacts. They may be asked to inform their close contacts of a positive test.

When to seek medical immediate attention

When to contact a health centre , working in healthcare or with vulnerable populations, testing after travel, safe at home, about close contacts, attribution, relevant links, media contact.

If you have tested positive using an at-home testing kit, you must call ProtectNWT to report your result. Self-reporting is a very important responsibility. Self-reporting means that public health can notify and protect other members of your community by identifying:

  • Close contacts
  • High-risk exposures sites (gatherings and closed in settings such as bingo halls, gyms, faith-based gatherings, long term care, household or private social events)
  • Other public exposure sites where all contacts cannot be identified or notified (bars, restaurants, grocery stores)

ProtectNWT is available 8 am to 6 pm, seven days a week by calling 811 (within NWT) and 1-833-378-8297 (outside NWT) and by email: [email protected] .

Call 911 or your health centre immediately if you are experiencing difficulty breathing (struggling for breath).

If you had a positive test and you are experiencing only these symptoms:

  • Shortness of breath
  • Severe fatigue or weakness
  • Severe sore throat or headache

If you have any other COVID-19 symptoms you find concerning you should make an appointment for follow-up at your clinic or health centre. Please call ahead to make arrangements and inform the staff that you’ve had a positive at-home test.

Higher risk

If any of the circumstances outlined below apply to you, it means you are at a higher risk. If you receive a positive at-home test result you must isolate immediately and contact public health or your health centre to arrange for testing and/or further assessment. Some people with risk factors may be eligible for certain targeted treatments only once they have had a confirmed positive COVID-19 test from a testing centre.

You are in a higher risk group if you are:

  • Not fully vaccinated (less than two doses of vaccine)
  • Younger than five-years-old
  • Older than 60 years old
  • Have had an organ transplant
  • Undergoing cancer treatment
  • Taking immune suppressing drugs
  • Taking heart or lung disease medicine every day
  • Diagnosed with severe kidney or liver disease
  • Suffering from dementia or have had a stroke
  • Obese (Body Mass Index more than 40)

People who work in a healthcare setting including long-term care facilities or with vulnerable populations (RCMP, shelter workers, correctional officers, daycare workers, or teachers) should contact a testing centre if they have a positive at-home test result.

You require a NEGATIVE Day 8 test from a healthcare provider to end self-isolation after travel if not fully vaccinated. Contact public health or your health centre to arrange for testing. Book in advance. Negative at-home tests do not allow you to end self-isolation early after travel.

The CPHO now considers all new cases in the NWT a result of the Omicron variant. The majority of people infected with the Omicron variant will safely recover at home. A small percentage will have severe symptoms from a COVID-19 infection that require medical attention. The chance of severe outcomes is higher if people are unvaccinated, elderly or immunocompromised.

It is important that people who have COVID-19 and their contacts isolate in place (where they are staying when notified) if possible. Omicron is highly contagious. It can spread to others 48 hours before symptoms appear and takes an average of three days to develop symptoms. Household contacts at the highest risk of exposure.

Any movement of persons who have COVID-19 increases the risk of transmission to others. People recovering from COVID-19 at home should isolate away from others as best as they can, get plenty of rest, stay hydrated, monitor symptoms, and follow healthcare provider advice (See: Isolating Safely at Home ).

People with COVID-19 may need help with groceries or other supplies while recovering at home. All communities should have plans in place to assist people who are isolating by dropping off necessary items outside. People with COVID-19 should not leave home without healthcare provider consent.

You are considered a close contact if you interacted with anyone with a suspected or confirmed COVID-19 infection while that person was contagious. This puts you at higher risk of acquiring COVID-19.

People who live in the same home as someone with COVID-19 are at increased risk of developing COVID-19 and are close contacts. Other situations, like being in the same car or room, or being close to someone with COVID-19 for at least 10 minutes, also means you are a close contact.

Sometimes, out of caution, it is necessary to consider all those who were at the same particular location or event as a person with COVID-19 as contacts because there just isn’t enough information to confirm one way or the other. However, very brief contact at offices or stores, especially where people are wearing well-fitted and well-constructed masks, are not considered close contacts.

CAR T-cell cancer therapy travel 'risky', haematologist warns, as families push for SA clinic

Sending critical cancer patients interstate for specialised treatment is both "suboptimal and risky", a leading haematologist has warned, as the South Australian government explores offering the treatment in Adelaide. 

Chimeric Antigen Receptor (CAR) T-cell treatment was developed in the 2010s and involves collecting a type of white blood cell from a patient, re-engineering them in a laboratory, then returning them to the patient's body where they multiply and attack cancer cells.

Adelaide haematologist Pratyush Giri said several South Australians had been referred to Melbourne for treatment, including in "particularly high volumes" from the Royal Adelaide Hospital (RAH). 

Patients had to travel interstate with a central line — a long tube — inserted deep into a large vein that allowed for treatment or the drawing of blood.

Dr Giri warned they could have a "significant medical event" if the line were pulled out.

He said there was also an assumption that the family of a CAR T patient would be able to help them live independently for at least a month interstate after a week in hospital, which was "often difficult with elderly patients with little support".

He said some of these patients subsequently declined the referral interstate and died from suboptimal treatment in SA.

"So we miss out on effective therapy for a few patients," Dr Giri said.

He said there was also a timing issue with CAR T therapy.

"We want to collect patients' [autologous] T-cells before they get exhausted by chemotherapy," he said.

"We have to opt for less desirable and unnecessary therapy as the timing sometimes doesn't suit the interstate centre."

He added that if a patient had an "adverse outcome" from treatment interstate, "retrieving the patient back to our state is not only a major undertaking but also a traumatic experience for the family."

Too little, too late

South Australian police officer Robin Firth died in Melbourne on November 2022 after being diagnosed with mantle cell lymphoma in January 2016 at the age of 46.

His wife, Dawn Firth, said he had reached a stage that year where he was told there were no further treatments available with their only option to travel to America for CAR T therapy.

After being told that without it "he wouldn't make Christmas", Mrs Firth said her husband wrote to SA politicians who took on his case and argued that patients should not be getting sent to America.

"CAR T was available in Melbourne at the Peter Mac (Peter MacCallum Cancer Centre), only it was for different kinds of cancer and Robin's wasn't one of them," Mrs Firth said.

"For four months we went backwards and forwards and eventually it was agreed that we could go to Melbourne for Robin to get the CAR T therapy. SA would pay 50 per cent and the Commonwealth would pay the rest."

By then, however, Mr Firth's situation had deteriorated due to delays in getting the treatment he required.

"We went to Melbourne in October. We were due to be there for eight weeks and unfortunately we were there just over four weeks when Robin died," Mrs Firth said.

She said his haematologist, Dr Giri, and the team at the RAH arranged for Mr Firth to be brought back to Adelaide after being told he would be moved to palliative care in Melbourne.

He had wished to be at "home" if the treatment did not go to plan.

"Unfortunately Robin died in the back of the ambulance that was transporting him to the air ambulance," Mrs Firth said.

"The trauma for our daughter Beth and I to watch Robin die in these circumstances away from home was beyond heartbreaking.

"After fighting so hard for seven years, this is not how it was supposed to end.

"No other families should have to battle the system to gain access to life-saving treatment and be expected to travel from home when they are already very unwell, taking them away from everything and everyone they know and trust."

Governments foot bill

CAR T treatment typically costs more than $200,000 and it is jointly paid for by state and federal governments, including for patients sent interstate to receive it.

Victoria, New South Wales, Queensland, and Western Australia offer the therapy in selected public hospitals.

About eight South Australians were referred interstate for the treatment in the 2022-2023 financial year.

An SA Department for Health and Wellbeing spokesperson said it had been "actively exploring the delivery of this highly specialised treatment in our state".

"The department is currently working with a number of agencies to develop an approach for evaluating the introduction of these therapies so we can make an informed decision on whether a local program can be viably introduced," the spokesperson said.

Petition launched

Adelaide woman Steph Walker recently launched a petition to bring the therapy to SA after her father was referred to Melbourne for six to seven weeks for CAR T-cell therapy.

"If anyone else has gone through a cancer journey, they'd know how much it can really destroy the strongest man you know," she said.

"It's quite tough on us, especially not being able to be with him during this time.

"We're going into this final treatment option with a 25 per cent success rate, so he might be spending his last six weeks there fighting for his life away from his family with no support system."

Her petition has received about 4,000 signatures after passing 3,000 in just five days.

The SA Commissioner on Excellence and Innovation in Health, Professor Keith McNeil, told ABC Radio Adelaide that the "basic knowledge and expertise" already existed in SA to offer the therapy.

"It's just a question of bringing it all together and making sure the laboratory's set up, the administration's set up, making sure all of those things are in place, and to make sure there are enough cases to justify doing it on an outcomes basis," he said.

He did not anticipate the cost being a big issue because SA was already paying for patients to go interstate for what was a "very expensive therapy".

"There would be some set-up costs, and reconfiguration and staff training to go into there, and that's part of the decision-making process," Professor McNeil said.

More vet services in Hay River, N.W.T., as Alberta clinic starts offering house calls

A new veterinary clinic will soon be offering services in Hay River, N.W.T., in the form of house calls and gas station meet-ups.

The Manning Veterinary Clinic said it is expanding its services to the community. Currently, there is no established, permanent veterinary clinic in Hay River, N.W.T., although one other Alberta-based clinic already travels there about once every two months.

Thaney Lupichuk is the practice manager at the Manning Veterinary Clinic. She said after the stress of all the floods and fires in Hay River, the decision for staff to start travelling there was a no-brainer. 

"We have many amazing clients from Hay River who usually come to us. By making regular trips up there, we can alleviate some of their travel burdens," she said.

Accessible care

They also want to ensure everyone has access to veterinary services, whether they can travel or not. Lupichuk said the bonus for clinic staff is exploring new places.

"We kind of see the opportunity for new adventures for us, too, and that's something we all thrive on here," she said. 

The clinic wrapped up its first trip with Veterinarian Dr. Jennifer McCracken earlier this week. They plan to return between Aug. 14 and 19 and then visit monthly. Until it can find a location to set up a clinic, it only offers house calls.

"We're looking for the most affordable and cost-effective option for establishing a physical place in town," Lupichuck said. 

Out-of-town clients can meet at another location in town, such as a hotel or gas station.

Lupichuk said expanding their services to the north is important because veterinary care in rural areas is often not easily accessible.

"We kind of felt that vet care in the north is challenging enough," she said. "Even if people from Fort Smith can make it to Hay River, it's much closer than travelling all the way to Manning."

While surgeries are unavailable until they establish a physical space, Lupichuk said the mobile service will cover wellness exams, vaccinations, and other non-surgical needs. Residents can call the Manning Vet Clinic for more info.

Long drive for life-saving services

Martina Jerome, who lives in Fort Resolution and breeds purebred Yorkies, said puppies must be vaccinated promptly, so she now vaccinates her dogs herself.

"Due to such distance, many people don't get their pups vaccinated and lose them to Parvovirus," she said.

There is also the issue of emergency situations. Not having access to a vet can be deadly for some pets in medical distress. With no emergency services available in Hay River, the closest emergency vet for Fort Resolution is in High Level, Alta., which is just over 450 kilometres away.

Jerome said she's also had some close calls with her pets and had to deal with the situation herself or fly to Yellowknife, which can be costly.

"[The] N.W.T. is desperate for vet services," she said. 

Jerome also hopes that with added vet services in the region, more people will get their pets vaccinated and spayed or neutered.

Yellowknife (January 10 th , 2021) – As the NTHSSA continues the response to COVID-19, services are being re-opened to the public. Yellowknife Primary Care Clinic (YPCC) will be reopening its Lab Services on January 10 th and Diagnostic Imaging Services on January 12 th , 2022.
